Welcome to the IS PACMaC Expression of Interest Form. 

Before completing the form, please ensure you have read through the 'Hosting a Course' details thoroughly. 

If your private or public hospital is interested in hosting an IS PACMaC, please complete the following form. We note that some of the details below may not be finalised at this stage but ask that they be filled out to the best of your ability. 

Once completed, the SPANZA Secretariat will reach out to the necessary facilitators to confirm the scheduled date and faculty. We will then be in contact to finalise the course details.
